How To Refer

We welcome referrals from clients, family members, advocates, deputies and solicitors.

If you would like to discuss a referral please telephone us or fill in the enquiry form below and we will contact you.

After a referral the following steps are taken:

• We will agree with you the best type of report for your needs. This can be a formal case management report , an immediate needs report , or a letter making recommendations.
• A quote for the initial assessment is sent to the referrer.
• A visit to the client will be carried out and a preliminary case management report or immediate needs report written.
• A clear time scales for a case management plan are identified.
• Current care and case management needs are costed.
• Equipment, therapy and miscellaneous needs are identified.
• We aim to finalise the report within three weeks of the visit.
